Ascent Health – Working with Children and Families in Hopes of Making a Difference

Host Jeremy C. Park talks with Jim Ward, State Director for Tennessee with Ascent Health, Inc., who shares some of the history for the nonprofit organization that was established in 2012 and has grown to provide services in three different communities, including Memphis, Tennessee. During the interview, Jim talks about their model and how they come alongside families, both youth and adults, providing care management, access to therapists, crisis response and support. He discusses their collaborative efforts and their work with schools, their home and clinic services, success stories, how the community can help, and more.

Ascent is a non-profit organization offering intensive, individualized care planning and management processes for children and adolescents (up to their 21st birthday) with complex mental health and/or other needs. These services are offered to young people who have involvement in multiple child-serving agencies (i.e. Office of Juvenile Justice, Department of Child and Family Services, Office of Behavioral Health, Department of Education) and whose families would thus benefit from coordination of efforts across these systems. Wraparound services are aimed at young people throughout Northeast Louisiana who, regardless of the system(s) in which they are involved, are at risk of placement in an out-of-home or alternative school setting, or who are transitioning back to the community from such placements.

We are driven by the many needs of the children and teenagers in Louisiana. Our passion to see families break cycles, move from conflict into unity, and turn hopelessness into empowerment has led many of our clients to improve their situations significantly.
